Why am I required to provide my email address during registration and sign in?

Lincoln Concierge Care Coordination uses email as the sign in “username”, eliminating the need for a separate member name. An email address is also essential for privacy protection, to contact you if there is a problem with your account, and to provide you with information about our service.


What if I forget my password?

If you cannot remember the exact password you chose when you registered, you can reset your password by clicking the “Forgot your password?” link under the sign in fields, and follow the prompts.


What is a Care Circle?

A Care Circle is an area of the Lincoln Concierge Care Coordination site designed to help provide communication between family and friends regarding a loved one’s independent living situation. The Care Circle is updated by the members and is used to build a support community around the patient. Care Circle members are encouraged to participate by creating and reading Care Posts, Observations, and Private Messages.


Why do I have to create a Nickname?

You will be prompted to create a Nickname to give you the option to protect your privacy. On the Lincoln Concierge Care Coordination website, your participation in a Care Circle will show with the name you selected.


Is the service free?

In order to be a full member you must be sponsored by your employer, health plan, or other organization. A full member can then invite family members and friends to participate for free on a limited membership basis.


How do I sign In?

If you have already registered with Lincoln Concierge Care Coordination all you need to do to sign in is enter the E-mail address you provided upon registration and the Password you created when setting up your account.


Who are the members of a Care Circle?

Care Circle members are the friends and family who visit a Care Circle to participate in conversations of support for the center (patient) of the Care Circle. For additional security, there is at least one administrator of a Care Circle. The administrators choose who to invite and what role they should participate (i.e. administrator or supporter).


Why do I need to register with Lincoln Concierge Care Coordination?

In order to sign into Lincoln Concierge Care Coordination website, you'll need to register with us. We will never share your personal information with a third party without your explicit permission. If you have any concerns about privacy, please read our Privacy Policy.


How long does my membership in Lincoln Concierge Care Coordination stay active?

Your Lincoln Concierge Care Coordination membership will remain active for as long as you'd like. Even if you were to lose sponsorship you can still participate indefinitely as a limited member.


How old do I need to be to use Lincoln Concierge Care Coordination?

Every Lincoln Concierge Care Coordination member must be at least 13 years of age to create a website account. In order to be sponsored full member you must be at least 18 years of age.
